How To Manage Post Acne Pigmentation

Advanced Facialist Mariam Abbas discusses the cause of acne-related pigmentation, and the most effective treatments to improve post-acne scarring.

Acne scarring and pigmentation are often emotionally distressing because they are so difficult to treat, and there can be a considerable amount of guilt associated with post-inflammatory hyperpigmentation (PIH). When acne treatment has cleared up the spots, if the skin doesn’t look clear and smooth (due to the scars) individuals can tend to continue treating their skin like acneic skin. I have had many clients approach me for acne treatment where the acne is long gone but the uneven skin texture has caused them to think they still have acne.

How Acne Related Pigmentation and Scarring Occurs

Post acne pigmentation is often a more difficult concern to treat than acne. Deeper, cystic acne lesions can leave both pitted or depressed scars on the skin where the skin tissue has broken down or raised scars where excessive collagen is produced. If acne lesions are picked and squeezed, they can leave pigmented scars. Acne pimples are like wounds and the trauma of squeezing a premature wound can cause post inflammatory hyperpigmentation.

Essentially post acne pigmentation and scarring means the skin hasn’t been able to heal properly from a wound and has left a stubborn mark.

Combination Approach To Treatment

There are many options to help with scarring, and treatments are becoming more sophisticated and effective, however, the most important aspect is managing patient expectations. PIH can be targeted with a combination approach, focusing on overall skin health to support a healthy skin metabolism and cell function alongside skin peels. It is important to create a synergistic environment and support system for cells while tackling the volatile pigment producing melanocytes. Micro-needling (sometimes with Radiofrequency), in combination with skin boosters, stem cell molecules and stimulating serums can help improve pitted scars. I like to alternate needling with Byonik Laser treatments which complement results by plumping the skin and stimulating collagen. Raised hypertrophic scars and keloid scars are best treated in medical clinics.”
